Greetings!

Thank you for your request dated Nov 14, 2022 11:35:02 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Board Certificate.

Response to Request

You may claim personally your board certificate to the concerned Regional Office at Releasing Counter, Monday to Friday (8 am to 5 pm) by presenting your valid PIC or any valid proof of identity. If through  representative, must present:
Authorization Letter (if representative is a licensed Professional) or Special Power of Attorney (if representative is not a licensed Professional).
