I trust this letter finds you well. I am writing to bring to your attention a matter related to record discrepancies that require prompt resolution.

 

I recently reviewed my records and noticed a discrepancy in certain details. In my Notice of Admission (NOA), driver’s license, and Leris account, my first name is consistently registered as Ma, without the period after "Ma." However, my philhealth ID and birth certificate includes the period after "Ma.”

 

I am particularly eager to resolve this matter promptly as the oath-taking ceremony is scheduled for next week. I already submitted the n
