Hello. Good day! 
May I request for a complete copy of my internship documents for:
-Community internship (Mercury Drug)
-Hospital Internship (Calamba Doctors Hospital)
-Manufacturing internship (Life Square Pharmaceuticals)
-Major Internship (Justice Jose Abad Santos General Hospital)

I took the board exam on August 30-31, 2017 and became licensed on September 2017, but I'm not sure when did PRC handed my documents to your office. I'm from CEU Manila.
