Good day

I am looking to renew my PRC license expiring this Aug 14 2022. The original details in my old card is Elaine Gannaban Ong,

I am able to login in LERIS with the former name. When I am trying to apply for renewal, it says 'record mismatched'. Please assist. Thank you. Other details can be seen 	at this email: technicalassistance@prc.gov.ph
