   I am KRISTA LYN F. URBANO living in Pato-o Odiongan Romblon. 
I humble to request to change or edit my birthyear in your system. I unintentionally put a wrong birthyear upon filing information form. Instead of 1997, I put 1996 when filing out the information form.
