I am a victim of identity theft. ARLENE JOY CASALAN-OJANO who is also a Pharmacist used my identity to file an LTO (license to operate) with the FDA to operate a pharmacy and is asking for monthly fees for the license to the owner of the Pharmacy without my knowledge and consent. She used my license and board certificate since August 2021 based on the information given to me by the FDA and I only found out about it on June 29, 2023 because I filed a variation with the FDA. I went to the pharmacy in Calamba, Laguna to find out and talk to the owner of the pharmacy.
