Hi,

In the process of renewing my CPA PIC via LERIS, I received  Record Mismatched. I noted that my last name in current PIC did match my last name in my LERIS profile.

My surname should be BOÑGOLAN instead of BONGOLAN.

I am unable to change the last name in my LERIS profile so i couldn't not proceed with the renewal. I have already submitted a query through https://crms.prc.gov.ph/feedbackform. Ticket number is Y4H2Z5WD.

Appreciate if somebody can help me resolve this issue.
