Hi All,

I am sending this request to seek assistance in obtaining authenticated documents (authenticated PRC License, authenticated certification of board rating, authenticated certification of passing) however I am getting a "RECORD MISMATCHED" prompt from the PRC website. 

PRC form is also requesting for the Notice of Admission w/c we are not sure since we don't have the said information available as of the moment. 

Applicant is Milagros S. Darcera RM.
