HI! I tried to apply for initial registration but the LERIS website states that I have record mismatched. I think it's because I was not able to enter my second name in my LERIS but my second name was written in my Notice of Admission

Kindly email your concern directly to technicalassistance@prc.gov.pd and provide your profession, license number, a valid ID (can be a PRC license), PSA (birth certificate), Notice of Admission(for initial registration and e-oath transactions) and the email address you have used for the PRC account for verification purposes. You may also proceed to nearest PRC office.
