Hi PRC,

I'm trying to renew my PRC ID/license online but I keep getting the error "RECORD MISMATCHED." 

I no longer have a copy of my Notice of Admission so I don't know  what is the mismatch. I have raised 2 tickets already in https://crms.prc.gov.ph but got no response. Tried calling the office locally but I'm also not getting any response.

For concerns related to PRC online services, you may raise it to https://crms.prc.gov.ph/feedbackform, if ever you still haven't received any response, you may proceed to nearest PRC regional office to personally verify the mismatched on your record and for the concerned office provide an immediate solution to your concern.
