A pleasant  day,

I would like to inquire if I could acquire a copy of my Notice of Admission.

I took the Licensure Exam for teachers January 2014 at Baybay Leyte. Unfortunately, I have lost already my NOA. I will try my luck to take the exam for the second time around since, I was not able to pass the exam that year. According to our PRC regional office no records found under my name in their office and they asked  me for a NOA.

Hoping for the positive response on this matter. Thank you in advance.
