Hi,
This is Michelle Anne Abalos , I have tried renewing nursing PRC ID online but received the "RECORDS MISMATCHED" note and I cannot proceed further. I have already checked and confirmed that I have keyed in the correct details based on the documents at hand.

I have also tried reaching out via email on the said technical assistance.

How do I rectify the mismatch and how do you get renewed? Is a personal appearance by a representative at the NCR office better in this case? Who do we contact?
