Good day,
I am trying to process an application for Authentication of my PRC ID but i get a
"record mismatched" popup. It says “The First Name and/or Last Name that you entered is/are
different from the details as printed on your Notice of Admission for Initial Registration transactions Professional Identification Card (PIC) for PIC Renewal.” I checked my information and it was all correct. I already emailed technicalassistance@prc.gov.ph but I still haven't received any reply.

Respectfully asking for some assistance regarding this matter.

I refer to my letter of Jul 07, 2022 01:19:52 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Authenticated PRC ID.

In order for us to work further on your request, we need more information from you regarding your query. Kindly respond to our email on July 14, 2022 and provide us the following for verification purposes.

1. Scanned copy/photo of your PIC (Professional Identification Card), another VALID ID and PSA issued Certificate of Live Birth.

2. Registered PRC online email account.

3. Nature of online transaction/s (renewal, certification, authentication etc.)

4. If you are transacting for CERTIFICATION/AUTHENTICATION OF RATING and PASSING, please provide us your date of examination and the name you used when you took and passed the examination


During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 07, 2022 06:08:27 AM, your request will be closed.
