SUBMITTED A PETITION FOR CHANGE OF NAME/STATUS, ALREADY VERIFIED ONLINE UNDER VERIFICATION OF LICENSE THAT MY NAME WAS CHANGED. UPON PROCEEDING TO RENEW ONLINE USING MY LERIS ACCOUNT, IT SAID THAT RECORD MISMATCHED SINCE THE NAME ON MY PERSONAL DETAILS IN LERIS IS STILL MY MAIDEN NAME. REQUESTING FOR ASSISTANCE TO CHANGE MY NAME ON MY PERSONAL DETAILS ON MY LERIS ACCOUNT TO RENEW MY PRC LICENSE
