Good afternoon I would like to request a second copy of my lupon or certificate of nurse, I got it way back 2015 February 16., It was lost, misplaced it and now that I am going abroad i need my second copy of lupon ng nurse and my prc license number is 0853502, i hope that you could hear my request and help me with this matter of importance, thank you.

Where to file and how much is the fee?

  Application for a duplicate copy of lost or destroyed Certificate of Registration (Board Certificate) can be filed at the Registration Division in the Central Office, and at the PRC Regional Offices.

What are the requirements for applying a duplicate copy of lost or destroyed Certificate of Registration (Board Certificate)?

A. The requirements are:
1) Duly accomplished Action Sheet for Registration Certificate Replacement
2) Affidavit of Loss/Damaged Certificate of Registration
3) Passport size picture with name tag in white background and application fee of P300.00
