I would like to request to change my first name that I entered on my leris account. It is different from the detail/s as printed on my Notice of Admission for Initial Registration transactions / Professional Identification Card (PIC) for PIC Renewal. I would like to correct my first name, it should be Ma. Eunice, the period after my name Ma is missing.
