Greetings!

I would like to verify if there is a board passer of Dentistry whose name is Thelma G. De Leon or Thelma Pili-Guanzon De Leon, who is born on March 24, 1965. I would like to request a Certificate that certifies if she is entitled or not entitled to do so or sign any document that implies that she is a Doctor.
