Hi! I can't make an appointment for renewal of my professional id card due to RECORD MISMATCHED on my PRC online profile specifically my middle name.

Wrong Middle Name: DIONISO
Correct Middle Name: DIONISIO

 I would like to ask for correction on my online profile from the details of my PRC license. 

Here are my details:
MYRA DIONISIO CORPORAL
lic.# 1515282
registration date: 12/23/2016
id card valid until: 09/05/2022

Kindly provide and send to prc.archivesandrecords2020@gmail.com your profession, license number, a valid ID (can be a PRC license), PSA (birth certificate), Notice of Admission(for initial registration and e-oath transactions) and the email address you have used for the PRC account for verification purposes.
