1. Birthdate shown is incorrect. The correct date must be January 19, 1985

- Kindly provide and send your profession, license number, a valid ID (can be a PRC license), PSA (birth certificate), Notice of Admission(for initial registration and e-oath transactions) and the email address you have used for the PRC account for verification purposes.
