Good Day!
Hi, I'm Princes Rabanes Maravilla, a bachelor in Secondary Education major in Filipino. 
 I made an account in Leris PRC and booked an online appointment for my application for Licensure Examination for Teachers this March 17, 2024 and approved my application and schedule that will be on Dec 22, 2023. However, only later I've found out when I checked my details, my surname and last name had switched, supposedly, my surname is Maravilla, but it became Rabanes, and my supposedly middle name is Rabanes but it came out as Maravilla. I need some assistance. 

I need help. Thank you
