You asked for REQUEST FOR BOARD CERTIFICATE.

Response to your request

Both the PRC ID and the COR(board certificate) are usually issued after the oath-taking ceremonies.
NOTE: No COR and PIC shall be issued to any successful examinee or applicant admitted for registration without examination unless he/she submits proof of having taken the Oath of Professional.

In case you haven't received on the day of oath-taking ceremony , please claim personally at the PRC office/branch you originally selected when you set an appointment for initial registration.
