Hi! I would like to seek your help regarding LOST Philsys TRN. I signed up for the PhilSys ID in Zamboanga City a year ago and lost the slip or document that contains my TRN number since I moved to Mandaluyong. Can you please help me recover my TRN ID?

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
