Hi! I would like to request for a replacement of my lost national id. I had lost it on Ocotober 27, 2023 due to pickpocket. I hope you can approve my request for it is the only identification i have aside from my school id.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
