I would like to request NATIONAL ID since I was lost mine 3 days ago.

I am hoping for your response, so I can know what are the documents needed for this.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
