Hi! I would like to request for my PhilSys ID since I lost my receipt after my registration in early 2022 and I still haven't received my National ID.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
