Good day,

I am Princess Hannah Bea Maramara, 17 years old, born on NOVEMBER 20, 2004. My mother is RUBELYN GRACE HUGOS MARAMARA, single mother at the time of my birth. I would like to please request a copy of my PSA-approved Birth Certificate for supporting documents for my application in USEP COLLEGE ADMISSION. I am humbly requesting for your approval and to release as soon as possible. Hoping for your kind consideration and understanding.

Please be advised that our CRS Outlets DO NOT accept walk-in clients and only clients with prior appointment can be accommodated. To secure an appointment, please proceed to appointment.psa.gov.ph.
