Hi there,

I would like to request a replacement physical card of my National ID as I have sadly lost it. I’ve been using my National ID as a form for verification.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
