Dear Sir,

I tried to get a copy of the birth certificate of my grandmother but due to some issues like missing parents name it was declined by PSA.

My grandmothers infos are:

Name: Cresencia Velos Awiten
Date of Birth: June 14, 1914
Place of Birth: Sagay Camiguin Island

The missing info is the given name of his parents:

Mother Family name: Velos
Father Family Name: Awiten

If you can help me finding the given name in your data base I would appreciate it.

This is a major concerns now in getting a docs from PSA where if one missing information is gone it will be decline automatically.

Our apologies but we are from the Philippine Statistics Authority.

Moreover, we regret to inform you that we do not have civil registry records from years 1945 and below.

We are also not authorized to verify a specific record due to RA10173 or the Data Privacy Act of 2012.

You may try to request for a manual verification in the Local Civil Registry Office where the documents were filed or to the National Archives of the Philippines.
