Hello! I would like to request for information regarding:

1. Total Number of patients with Chronic Kidney Disease in San Jose and Lipa City, Batangas for the past 10 years. 
2. Total Number of patients with kidney cyst in San Jose and Lipa City, Batangas for the past 10 years. 
3. Total Number of patients with kidney inflammation in San Jose and Lipa City, Batangas for the past 10 years.
4. Total number of Diabetic patients in San Jose and Lipa City, Batangas for the past 10 years. 

NOTE: Please INCLUDE patients who are not receiving dialysis treatment but diagnosed with these diseases.
