Hello, I would like to request for the transaction number of my national ID. I really needed it in case I would browse or make a request of an electronic copy of my national ID.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
