Hello, I would like to know if a scanned copy of authorization letters and ID are enough to get their PSA birth certificates? They both live abroad.  Also, my cousin is already married there which shows her married name and not her maiden name.

You asked for Requesting birth certificates in behalf of Aunt and cousin.

Your FOI request is approved. Our apologies for the delayed response. In requesting civil registry certificates on behalf of one person, she/he may authorize a representative to request on your behalf, kindly provide the following upon application/claim of the document:

a. Original valid ID or photocopy of valid ID of the document owner and original and photocopy of the valid ID of the representative

b. Authorization letter / Special Power of Attorney (SPA) duly signed by document owner and indicating the following:

Type of document
Number of requested documents
Name of the document/s; and
Complete details of requested documents
