To whom it may concern,

I hope this email finds you well. I am writing to inform you that I have unfortunately lost my identification card, Phil ID, bearing the ID number, 2392-7604107-5 with the tracking number of 10317213440575620210706075031.

Attached in here is my request and communication to the EFOI. I am requesting for an update about my declaration ( last September of this year) for the reason that I need a valid Identification for my daily life.

I understand the importance of ensuring the security of my personal information, and I am willing to provide any additional details.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
