Hi I would to request another National ID Card because it was lost. and also I am afraid that someone use it for illegal purposes.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
