Hello, Good day! 

I would kindly like to ask help about my mom's birth certificate for her to be enrolled as an HMO dependent of my company health insurance benefits. My mothers maiden name is Galilea Quisel Binghay, born on November 06,1965. She's married to my dad now. My mom visited our PSA branch here in cebu to get a copy of her birth certificate, resulting to no record per file. We couldn't get help from Civil Registrar's Office because my mom was born in Dipolog City, PH. Her dads name was Vicente Reyes Binghay, her late mom: Prudencia Quisel Binghay. My mom barely remembers all info

For this concern, you may request for a manual verification in the Local Civil Registry Office in the city/municipality where the birth certificate of your mother was registered.
