Hi! I would like to request a copy of my Philippine National ID claim stub and transaction number.

Unfortunately, because it has been a long time since the date of registration, I lost the claim stub and transaction/reference number given after Step 2. 

Because of that, I cannot claim my printed national ID because the personnel there told me that I need that transaction number given in the date of registration.

May you please email me a copy of my claim stub, if applicable, at jemimamondares@gmail.com.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
