Hi! I would like to request a copy of my Philippine National ID claim stub. Unfortunately, because it has been a long time since the date of registration, I lost the claim stub. May you please email me a copy of my claim stub, if applicable, at cheemaecandelaria@gmail.com. Thank you!

Greetings!

Thank you for your request dated Jan 19, 2024 08:59:42 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for I want to have a copy of my Philippine national id stub.

Your FOI request is approved. 

In case you lost your transaction slip, you may present any valid ID on the day of your PhilID delivery.

However, if you are requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
