Good day!

This pertains to the PhilSys ID of my parents, Nestor Malinao Pradilla and Jacinta Amista Pradilla. I understand the the Transaction Slip is needed to claim the physical ID at the post office. However, theirs was inadvertently misplaced after we move documents from our previous dwelling to our current address. I exhausted all efforts possible in the hopes of locating such, but to no avail.

Hence, we are earnestly pleading if we may be given copies of the slips via email, so we can proceed in claiming their IDs. My email address is mitchelle.pradilla@dhsud.gov.ph
