I would like to request a replacement for my lost/stolen national ID. I am in need of government issued ID for personal, school, and document purposes.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
