Hello, good afternoon!

I would like to report that I have lost my National ID yesterday in the premises of the Philippine Arena. I have reported it to the said place's lost and found corner but nothing has been surrendered to them at the time of my report. They got my information so that they can contact me if someone surrenders my ID but no one has reached out almost 24 hours since it got lost. 

May I ask for assistance in my request to get a replacement for my lost ID and what will be my requirements.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
