I would like to request a replacement of my lost PhilSys ID. Please let me know what else I need to do to grant my request. I still have my PSN, it’s just the card that I lost.

Your FOI request is approved. 

In reference to your concern, kindly wait for the announcement in our official channels for the process of replacement of lost/stolen PhilIDs.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
