Hi! I would like to request a copy of my Philippine National ID claim stub.

Unfortunately, because it has been a long time since the date of registration, I lost the claim stub.

2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
