Hi,

Good day.

I would like to ask if I can get a replacement for my lost National ID Card. My bag was stolen while attending the mass last October 15, 2023 at a church here in Talisay City, Cebu. I was able to keep a record of my PSN.

Please let me know what are the requirements. Your response is greatly appreciated.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
