1. National ID- It has been lost for almost 10 months now and the baranggay Silangan, Cantabaco, Toledo City, Cebu 6038 has not given any actions to it.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
