Good day, I would like to request for a new National ID because I lost it with my wallet. Now, if a request for new national ID is not available. I would like to request for my Transaction Slip because I misplaced it and cannot find it anymore. I will use it to request for an ePhil ID

Thank you for your request dated Jan 14, 2024 12:42:40 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You asked for Lost of National ID and Transaction Slip.

Your FOI request is approved. 

Kindly wait for the announcement in our official channels for the process of replacement of lost PhilIDs.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.

As for your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address info@philsys.gov.ph:

1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
