Dear Maam/Sir,

I hope this message finds you well. I am writing to request a replacement for my lost national ID card. Unfortunately, I have misplaced my original ID, and I understand the importance of having a valid national ID for various official purposes. I am willing to go through necessary steps in order to do so. Thank you for understanding!

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
