Dear Sir/Madam,

Good day!

Requesting to your good office of the procedure on correction of birth certificate as there was a typographical error on my Surname.

Last month, I've already submitted the necessary documents to the Local Civil Registry at Calamba Municipal Hall. May I know the next legal process to claim my certificate of finality? Also, how will I get my annotated birth certificate? Can I order it online or I should go to PSA main office or regional office?

I appreciate your prompt feedback.
