Good day Ma'am/Sir,

Hi, I'm Keil Ezon H. Gan, a graduating student of a local city college, and humbly asking to your good office to allow me to avail a new ID since I've lost it accidentally. As a graduating student it is really essential for me to have a valid government ID.

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
