I would like to request a new Philippine ID card since I lost mine. 

I badly needed it as it is one of my valid id

Please note that you need to only register ONCE in the PhilSys; with the issuance of your PhilID, your PhilSys Number (PSN), which is your permanent unique identification number in the PhilSys registry has already been generated.
