To request a manual verification to our Civil Registry Service on your processed documents, please send the scanned copy of the transmitted documents, as listed through the following email addresses: crs.qvu1@psa.gov.ph,crs.qvu2@psa.gov.ph, crs.qvu3@psa.gov.ph:

     1) Certified True/photocopy of the Court Decision;
     2) Certified True/photocopy of the Certificate of Finality;
     3) Certificate of Registration of the Court Decision;
     4) Certificate of Authenticity of the Court Decision;
     5) Certificate of Marriage Without annotation; and
     6) Certificate of Marriage With annotation.
