Hi, I would like to inform you that I have lost my National Identification Card while I’m riding a jeep around Malabon City, I am hoping to replace it and get another one, by any chance what are the requirements to get a replacement for my lost ID. I fear that my ID can be used on crimes such as fraud that’s why I immediately sent this message.

Thank you and I am Hoping for your favorable response!
